About the Role:

Join HomeWell Care Services CA305 as a Care Manager RN, where you'll play a vital role in enhancing the quality of life for our clients. We are looking for a compassionate and skilled RN to lead our care team and ensure exceptional service delivery in Upland, CA.

Responsibilities:
  • Assess client health needs and develop personalized care plans.
  • Supervise and support caregivers in delivering high-quality care.
  • Conduct regular client evaluations and adjust care plans as necessary.
  • Collaborate with healthcare professionals to coordinate services.
  • Ensure compliance with state regulations and company policies.
  • Provide education and support to clients and their families.
  • Maintain accurate documentation of client progress and care activities.
  • Participate in ongoing training and professional development opportunities.
Requirements:
  • Current RN license in California.
  • Minimum of 2 years of nursing experience, preferably in home health or hospice.
  • Strong leadership and communication skills.
  • Ability to develop and implement effective care plans.
  • Compassionate demeanor with a focus on patient-centered care.
  • Proficient in electronic health record (EHR) systems.
  • Valid driver's license and reliable transportation.
  • CPR and First Aid certification required.

Compensation: $95,000.00 - $115,000.00 per year
