Join HomeWell Care Services CA305 as a Care Manager RN, where you'll play a vital role in enhancing the quality of life for our clients. We are looking for a compassionate and skilled RN to lead our care team and ensure exceptional service delivery in Upland, CA.
Responsibilities:
- Assess client health needs and develop personalized care plans.
- Supervise and support caregivers in delivering high-quality care.
- Conduct regular client evaluations and adjust care plans as necessary.
- Collaborate with healthcare professionals to coordinate services.
- Ensure compliance with state regulations and company policies.
- Provide education and support to clients and their families.
- Maintain accurate documentation of client progress and care activities.
- Participate in ongoing training and professional development opportunities.
- Current RN license in California.
- Minimum of 2 years of nursing experience, preferably in home health or hospice.
- Strong leadership and communication skills.
- Ability to develop and implement effective care plans.
- Compassionate demeanor with a focus on patient-centered care.
- Proficient in electronic health record (EHR) systems.
- Valid driver's license and reliable transportation.
- CPR and First Aid certification required.

Compensation: $95,000.00 - $115,000.00 per year
