PrimaryOne Health is currently seeking Licensed Practical Nurses for our Columbushealth centers.

Essential Job Responsibilities:
  • Provides professional nursing care for adults, prenatal and pediatric patients. Includes intake, triage, patient assessment, history, vital signs, injections and outpatient procedures; performs vein puncture, EKG administration when needed; and patient education. Includes case management.
  • Maintains communication with patients concerning treatment progress and diagnostic reports. Coordinates care with other health services team as appropriate.
  • Identifies and coordinates referrals to community resources in regards to patient needs.
  • Documents all professional activities, contacts and observations promptly and appropriately.
  • Attends internal and community meetings as assigned and participates in Quality Improvement activities as assigned.
  • Participates in case management activities.
  • Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.
  • Occasional travel required.
  • Other duties or special projects as assigned.

Successful candidates must possess a high school diploma and a valid license as a practical nurse in the state of Ohio. Two years experience in ambulatory care and public health setting preferred.
