LPN Responsibilities:

  • Document nurses' notes in a clear, informative manner that accurately reflects the resident's care and their responses.
  • Maintain thorough recordkeeping by completing and filing necessary forms upon resident admission, transfer, and discharge.
  • Prepare and administer medications as prescribed by physicians.
  • Ensure that certified nursing assistants (CNAs) understand and follow resident care plans during daily care routines.

LPN Requirements:

  • Possess a Nursing Degree from an accredited college or university and be a graduate of a Connecticut approved RN/LPN program.
  • Hold a current certification as a Nurse Aide.
  • Communicate effectively in English, both verbally and in writing.
  • Ability to lift, push, pull, and move a minimum of 40 pounds to a height of 3 feet and move such weight a distance of at least 4 feet.
