- Licensure: Current PT license and registration in New York.
- Experience: Proven experience providing adult PT, preferably in a home care setting (though not required).
- Communication Skills: Strong interpersonal skills to build rapport with patients and collaborate with office staff.
- Tech-Savvy: Familiarity with EMR systems is a plus (we provide training).
- Patient Care: Conduct evaluations and create personalized therapy plans to help patients regain mobility and independence.
- Flexible Scheduling: Coordinate visits directly with patients, choosing when and where you want to work.
- Care Planning & Follow-Up: Develop and implement care plans, with regular follow-up visits to monitor progress and ensure continuity of care.
- Accurate Documentation: Record all patient interactions in our easy-to-use, web-based EMR system, ensuring compliance with Medicare and company guidelines.
- Compliance: Maintain your credentials and adhere to HIPAA and company policies to ensure the highest standards of care.
