Duties and Responsibilities:
- Conduct real-time mental health and substance use risk assessments.
- Develop and implement individualized crisis treatment plans.
- Provide evidence-based care coordination to prevent hospitalizations.
- Collaborate with clinical supervisors for skill training and crisis intervention services.
- Ensure accurate documentation in confidential patient records.
- Strong communication skills and ability to work with diverse populations.
- Team player with a focus on collaboration and patient-centered care.
- Ability to handle multiple projects and sensitive inquiries.
- Basic knowledge of suicidal/homicidal ideation warning signs.
