Working to meet the health and nutritional needs of women, children and families with information, education, and quality care, MFHS is currently seeking a per diem Licensed Medical Social Worker/Counselor for our Scranton Circle of Care location.

JOB SUMMARY

The Licensed Medical Social Worker/Counselor position is responsible for the diagnosis and treatment of mental and emotional disorders, including disorders of mood and addiction, aimed at the prevention and management of such disorders; provides screenings for trauma and domestic violence; and consultation and psychotherapy for individuals, couples, and families. The position requires emotional resilience and stability since the work often involves dealing with patient crises. The Social Worker will serve as both an advocate and therapist to help patients gain access to resources while confronting their personal issues/struggles and barriers, such as mental illness, addiction, and other life stressors.

Requirements

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

• Counsels' clients to help with mindfulness, manage crisis situations, define treatment goals and plans for recovery.

• Responsible for connecting patients to supported services and programs, including but not limited to, arrangement for comprehensive pre-natal care, labor and delivery follow up care, post-partum services including contraception and gynecological/family planning services, the Women, Infant and Children (WIC) program, referrals to the Nurse Family Partnership and other support programs as required.

• Conduct hospital visits for high-risk maternity patients after delivery to be certain care plans and post discharge medical/social service referrals are executed, barriers identified and resolved, and referral appointments are kept.

• Provide screenings to identify high risk clients and assist clients to navigate systems of treatment and care in accordance with policies and procedures of MFHS programs and peripheral services including, but not limited to, maternity pre-natal care, family planning services and counseling, pregnancy testing and antepartum and postpartum care.

• Participates in case conferences and provides case management and/or brief therapy to identified clients who present with depression anxiety, or other at-risk mental health behaviors as appropriate.

• Conducts diagnostic biopsychosocial assessments, individual and group family therapy and substance abuse, trauma, domestic violence screenings, and develops comprehensive treatment planning with crisis plan development.

• Consults and works in collaboration with other MFHS counselors, nurses, and providers to educate and increase their capacity to identify mental health issues with their clients, and collaborate with the MFHS Reproductive Health Educator to increase knowledge to provide education tools to Family Planning patients and seek additional reproductive health outreach and education opportunities within the community.

• Strengthen community collaboration with area mental and physical health providers to increase referral sources to and link clients to a higher level of service as needed. Coordinate referrals ensuring appropriate information is shared while securing system privacy standards.

• Promote and contribute to organizational and programmatic goals of MFHS.

MINIMUM QUALIFICATIONS REQUIRED

• Master's degree in social work with LCSW, LSW (if LCSW eligible) and LPC credential

• Bilingual - Spanish/English preferred

• 3-5 years' experience

• LCSW, LPC, or LSW (if LCSW eligible) license

• Drivers' license and insured automobile

• Board certified in Perinatal Mood disorders preferred

• Basic concepts and principles of managed care

• Trauma informed care

• Family and Group Dynamics

• Knowledge of community resources

• Inductive reasoning - ability to arrange things or actions in a certain order or pattern according to specific rules or set of rules.
