Physical Therapist (PT) in Skilled Nursing Facility

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Hourly Pay Rate: $42-$46 Hourly (W-2), $55-$57 Hourly (1099)

Physical Therapist Job Duties:
• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

Physical Therapist Requirements:
• Current NYS PT License and Registration
• CPR Certification
• EMR Competence

Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
