Winchester, KY, United States Healthcare Staffing Plus Full time

Shifts: either (5) 8 hr shifts or (4) 10 hr shifts, weekends will be one major and 1-2 minor holiday and backfill if needed for vacation fill-in.

1 - 2 days of call per week. VERY Rarely needed to come on site, just an occasional phone call

Essential Functions

  • Manages drug therapy protocols as a part of an interdisciplinary team.
  • Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug related problems.
  • Provides drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students.
  • Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
  • Evaluates medication orders for accuracy and completeness prior to order entry. Checks for accuracy after filling by technicians.
  • Maintains adequate knowledge of pharmacy automated systems and computer software.
  • Ability to manage patients in an outpatient anticoagulation clinic, including operation of Point-of-Care testing, evaluation of results, dosing and scheduling.

QUALIFICATIONS
  • Graduate from school of pharmacy or active license to serve as proof of education-
  • Doctor of Pharmacy - Preferred
  • Minimum 1 year hospital pharmacy experience or completion of PGY1 residency - preferred
  • Requires critical thinking skills, decisive judgement & the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action
  • Basic Life Support (BLS) obtained within 10 days of hire; Advanced Cardiovascular Life Support (ACLS - obtained within 30 days of hire;
  • LICENSE: KY Board of Pharmacy in good standing

• Effective interpersonal/communication, organizational, and the flexibility to handle multiple tasks.
