  • Shift: 8-hour days, Monday-Friday (5 shifts per week)

Social Worker Job Requirements
  • Minimum 1 year of skilled Home Health experience (home health social worker jobs)
  • Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW) preferred
  • Active Oregon social work licensure required; Washington licensure can be in process
  • BCLS certification required
  • OASIS experience required
  • COVID-19 vaccination (primary series plus one booster) required
  • Reliable transportation for daily travel (up to 60 miles radius)
  • First-time travelers accepted
Social Worker Responsibilities
  • Deliver case management and social work services to home health patients
  • Conduct patient visits (up to 6 per day) and meet productivity standards
  • Assess patient needs, develop care plans, and coordinate healthcare resources
  • Document care using EPIC Remote Client and Rover/OASIS
  • Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
  • Maintain professional attire (business casual or scrubs)
