About the Role
In your role, you will conduct thorough assessments during visits that can last up to one hour. By reviewing each member's health, medical history, medication adherence, social environment, and other risks, you'll gain valuable insights that are often missed in standard office appointments. Your efforts through the Matrix Comprehensive Health Assessment (CHA) will significantly elevate the quality of care and help bridge multiple care gaps in just one visit. Your responsibilities will include:
- Carrying out adult/geriatric assessments, including medical history, diagnosis, treatment, health education, and referrals, all adhering to Matrix guidelines.
- Administering and ordering necessary point of care and lab-based testing.
- Collaborating with physicians, case managers, social workers, families, and caregivers.
- Working alongside Primary Care Physicians (PCPs) for patient education and follow-up.
- Providing care directly at members’ homes or Skilled Nursing Facilities.

- Flexibility - Schedule visits based on your availability We conduct assessments seven days a week from 8 AM to 7 PM, allowing you to choose daytime, evening, or weekend slots that fit your schedule.
- Competitive Pay - Your earnings will reflect the number of assessments you complete. With greater availability and travel, those averaging 20 - 100 visits a month can earn between $1,800 - $11,500, based on our completion rate of $90 - $115 per assessment.

- Additional Licensure - If needed, we support you in obtaining credentials in additional states to widen your ability to serve members.

Skills & Experience That Shine
- Master's Degree or equivalent experience with successful NP licensure.
- Current NP licensure in your practice state with prescription authority or the ability to obtain it.
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Family, or other applicable specialties.
- Current BLS, ACLS, or CPR certification.
- Preferred: 1 year of NP experience, but new graduates are welcome to apply
- Valid state driver’s license and adequate automobile insurance.
- Strong computer aptitude and knowledge of medical record software.
- Excellent verbal and written communication skills.
