Case Manager

Location: Hartford & Tolland Counties, Connecticut
Travel: 100% (between 3 facilities in Hartford and Tolland counties)
Duration: 4-6 months
Schedule:
- Shift: Monday-Friday, 8:00 AM - 5:00 PM
- Dress Code: Medical attire (scrub color not specified)
The Registered Nurse (RN) will provide skilled nursing care and support for the Institutional Special Needs Plan (ISNP) program. This role involves coordinating care with providers and interdisciplinary teams, ensuring accurate documentation, and assisting with benefit determinations. The RN will manage patient changes in condition, participate in facility-based meetings, and support therapy coordination. Responsibilities also include conducting in-services, managing acute and chronic conditions, and ensuring compliance with quality measures.
Primary Responsibilities:
- Ensure accurate and complete documentation.
- Communicate and collaborate with interdisciplinary care teams.
- Assist providers with care coordination and Medicare benefit determinations (Parts A, B, D).
- Respond to patient changes in condition in collaboration with APCs and facilities.
- Support therapy coordination and participate in facility-based meetings.
- Schedule and participate in family conferences, team meetings, and case presentations.
- Coordinate training, compliance tracking, and quality measures.
- Conduct in-services for selected audiences.
- Assist providers with documentation, coding, medication reconciliation, nursing rounds, and assessments.
- Write verbal orders in compliance with state RN practice laws and facility policies.
- Manage members through acute medical situations or chronic condition exacerbations.
- Support members during short-term sub-acute rehabilitation episodes.
- Active and unrestricted RN license in state of residence (or ability to obtain by start date).
- Associate degree or higher in Nursing.
- Certified in Basic Life Support (BLS).
- Ability to lift up to 30 lbs and perform physical assessments.
- Access to reliable transportation.
- Full COVID-19 vaccination (proof required).
- Must pass drug screening (UnitedHealth Group is a drug-free workplace).
- 1+ year of clinical experience (long-term care setting preferred).
- Experience with patients with complex behavioral, social, or functional needs.
- Knowledge of geriatrics and chronic illness management.
- Proficient in electronic medical records and documentation.
- Strong communication and time management skills.
- Ability to work with diverse care teams across multiple facilities.
- Active RN license.
- Basic Life Support (BLS).
- Associate degree or higher in Nursing.
