Job Opportunity
Position OverviewWe are seeking a dedicated and experienced Registered Nurse specializing in Case Management to join our team at an acute care facility located in City, State. The ideal candidate will have a minimum of 2 years of acute care hospital experience, specifically in Care Coordination and Discharge Planning.
Key Responsibilities- Perform comprehensive care coordination and discharge planning for patients.
- Assess patients' needs and develop individualized treatment plans.
- Utilize InterQual criteria for admission and care planning.
- Organize and participate in family meetings to discuss patient care.
- Coordinate with interdisciplinary teams, including Social Services and Transportation.
- Arrange for durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport.
- Navigate regulatory requirements, including compliance with CMS standards and understanding DRG classifications.
- Must possess a valid RN license in the state of California, or have a pending application with active licensing by the start date.
- Minimum of 2 years of acute care hospital case management experience.
- Experience with care coordination and discharge planning is essential.
- Knowledge of drug and alcohol abuse treatment options, mental health crisis intervention, and advance care directives.
- Strong organizational and communication skills, with the ability to work independently and in a team environment.
- Proficient in computerized documentation, specifically with Epic, and competent with Microsoft applications.
This position requires a day shift of 5x8 hours (08:00 AM - 04:30 PM). Weekend coverage is expected every other weekend to meet departmental needs.
Orientation and SupportNew hires will complete a 40-hour orientation. The role includes support from a diverse interdisciplinary team.
Patient DemographicsThe case manager will work with a varied patient population, including adolescents, adults, and geriatrics.
