As a Clinical Management Associate, you will play a vital role in supervising and training primary care providers (PCPs) in your center, ensuring excellent execution of our core model. You will also directly provide patient care, balancing your time between leadership responsibilities and clinical duties.
Key Responsibilities:
- Provide comprehensive care for patients with acute and chronic illnesses, particularly older adults.
- Take full accountability for patient care and outcomes, engaging specialists as necessary while remaining involved in ongoing care.
- Collaborate with hospitalists for patient care coordination during hospital stays.
- Manage assessments, diagnosis, treatment, care coordination, and health promotion for patients with complex health needs.
- Lead a care team, including care promoters and facilitators, to optimize patient care.
- Engage with transitional care teams for patients unable to come to the office.
- Facilitate Super Huddles and Transforming Care Meetings to drive clinical excellence.
- Assist with center management, including patient recruitment and provider interviews.
- Mentor and coach PCPs to achieve outstanding clinical outcomes.
- Collaborate with Center Operations and Market General Managers to enhance financial performance and increase center membership.
- Perform other duties as assigned.
Knowledge, Skills and Abilities:
- Proficient in Microsoft Office Suite and a variety of clinical applications.
- Willingness to travel locally for work and attend national training sessions.
- Strong verbal and written communication skills; bilingual capabilities beneficial.
- Ability to exercise independent judgment effectively.
Education and Experience Required:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current MD licensure in the relevant state.
- Minimum of 1 year of clinical experience preferred, particularly in a geriatric or family practice setting.
- Completion of required ChenMed training programs.
- Board certification or eligibility in applicable specialties.
- Must hold a current DEA number and possess Basic Life Support certification within the first 90 days of employment.
Salary Range:
$231,876 - $331,251
