As a leader in this vital field, you will report to the Executive Director or Administrator and oversee patient care activities, ensuring high-quality and compliant hospice services. Your Key Responsibilities:

  • Direct and oversee patient care services, guaranteeing the highest standards of hospice care.
  • Manage patient visit scheduling, utilizing your clinical judgment to prioritize reassigned, missed, or rescheduled visits.
  • Provide immediate assistance by triaging incoming phone referrals and medical concerns with professionalism and compassion.
  • Lead Interdisciplinary Group (IDG) meetings by presenting patient updates and facilitating care planning discussions.
  • Act as a mentor and clinical resource, fostering the growth and compliance of your team.
  • Ensure all patient care meets the necessary regulatory standards and aligns with company policies.
  • Engage in quality improvement initiatives and operational efficiencies at your location.

What You Bring:

  • Valid RN license in your state of employment.
  • Graduate from an accredited nursing program.
  • A minimum of 3 years of direct patient care experience, including 2 years in hospice or home health settings.
  • Proven ability to implement hospice principles and ensure regulatory compliance.
  • Compassion for terminally ill patients and their families.
  • Ability to effectively lead a diverse care team and adapt to various patient needs.
  • Strong organizational, interpersonal, and leadership skills.
  • Proficiency with clinical documentation and technology systems.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN).
  • Previous leadership experience in hospice or home health.
  • Familiarity with EMR systems and telehealth platforms.
  • Certification in Hospice and Palliative Nursing (CHPN).
