Join our compassionate hospice team as a Patient Care Manager, where you will play a pivotal role in advancing hospice care while inspiring excellence in clinical practices.
As a leader in this vital field, you will report to the Executive Director or Administrator and oversee patient care activities, ensuring high-quality and compliant hospice services. This is your opportunity to manage clinical operations, coordinate multifaceted care plans, and develop staff dedicated to delivering the best patient care in a respectful and dignified manner.

Your Key Responsibilities:
- Direct and oversee patient care services, guaranteeing the highest standards of hospice care.
- Manage patient visit scheduling, utilizing your clinical judgment to prioritize reassigned, missed, or rescheduled visits.
- Provide immediate assistance by triaging incoming phone referrals and medical concerns with professionalism and compassion.
- Lead Interdisciplinary Group (IDG) meetings by presenting patient updates and facilitating care planning discussions.
- Act as a mentor and clinical resource, fostering the growth and compliance of your team.
- Ensure all patient care meets the necessary regulatory standards and aligns with company policies.
- Engage in quality improvement initiatives and operational efficiencies at your location.
What You Bring:
- Valid RN license in your state of employment.
- Graduate from an accredited nursing program.
- A minimum of 3 years of direct patient care experience, including 2 years in hospice or home health settings.
- Proven ability to implement hospice principles and ensure regulatory compliance.
- Compassion for terminally ill patients and their families.
- Ability to effectively lead a diverse care team and adapt to various patient needs.
- Strong organizational, interpersonal, and leadership skills.
- Proficiency with clinical documentation and technology systems.
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN).
- Previous leadership experience in hospice or home health.
- Familiarity with EMR systems and telehealth platforms.
- Certification in Hospice and Palliative Nursing (CHPN).
