Shift: M-F 8-4:30 rotating weekendsScrub Color/Attire - Grey scrub top and black scrub bottomsNo floatingWeekend coverage needed - Weekend Requirements: 1-2 weekends a monthOn Call Requirements NoneEMR MeditechType of equipment Bioness; FES; BiodexPatient Ratios 4-5/1Number of Beds - 15Utah Physical Therapy License; BLSPositive, upbeat, encouraging; engaging; compassionate; motivatingPhysical therapy evaluations with varying diagnoses75-90 minutes physical therapy treatment plans; 25-30% of our patients are stroke/neuroTeam player with all disciplines-PT/OT/ST/SW/RN/MD/CRS/PPSCDeveloping CARE scoring with all patients for quality/reimbursement purposesNumber of Staff 3 FT PT; 2 PRN PT; 4 FT OT; 2 PT OT; 3 PRN OT; 1 PRN COTA; 1 FT ST; 1 PRN STDept: IP Rehab Physical Therapy
Required Qualifications
Physical Therapist, Rehab
Experience: 2 years
PT(Compact Accepted)
Certifications: BLS-AHA
SSN Required
DOB Required
Work History: Should not be more than 90 days of gap in last 1 year
Good organizational and Communication skills working with the many disciplines
