Home Visit Specialist - Part-Time

Concentric (CHI) is seeking dedicated and compassionate Home Visit Specialists to join our team. In this role, you will conduct in-person home visits to build relationships, identify barriers to student success, and serve as a bridge between families, schools, and districts.

As a Home Visit Specialist, you will:
• Conduct in-person home visits to connect with families and provide crucial support.
• Use communication and relationship-building skills to foster trust and engagement.
• Share important school and district resources with families.
• Provide reminders about key deadlines, missing documents (e.g., immunization records), and deliver materials such as welcome packets, devices, and food when needed.
• Document all home visit outcomes in the Home Visit and Student Engagement App.
• Attend pre- and post-visit meetings with school and district personnel as needed.
What We're Looking For
Required Qualifications:
- Strong written and verbal communication skills.
- Valid driver's license and a clean driving record.
- Valid car insurance and reliable transportation.
- Cell phone and/or tablet with internet access.
- Reliable, punctual, and self-motivated professional.
- Availability to work evenings (no later than 8:00 PM) and weekends.
- Able to work a minimum of 20 hours per week.
- At least an Associate's Degree in a related field.
- Experience in home visits, documentation, and case management (e.g., Social Worker, Behavioral Specialist, Probation Officer).
- Comfortable using technology (Google Meet, Google Docs, Google Sheets, Zoom, etc.).
