Responsibilities

What You'll Do

  • Deliver therapy and wellness services (physical, occupational, or speech) tailored for older adults.
  • Conduct comprehensive assessments, formulate personalized care plans, and implement evidence-based interventions within a collaborative team environment.
  • Facilitate care coordination through wellness check-ins, effective communication with caregivers, and telehealth support.
  • Adhere to professional and ethical standards as aligned with company initiatives.
  • Contribute proactively to program development and continuous quality improvement.
  • Enable residents to enhance their independence, confidence, and satisfaction.
