Shift Information: Monday-Friday 8a-5p
On call: Once every 5 weeks

What You'll Do:
- Conduct timely psychosocial assessments of patients and families in the hospice program, ensuring that identified needs are addressed in the care plan
- Provide exceptional service to patients, families, referral sources, and co-workers, while assisting team members as necessary
- Offer counseling to patients, families, and staff on stress management and coping strategies
- Document patient services accurately and promptly to reflect the care plan
- Assist with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver, sourcing resources from community agencies to meet patient and family needs
- Act as a liaison between patients, families, and community agencies
Qualifications:
- Bachelor's degree in Social Work required; Master's degree preferred
- Current Virginia State Licensure in Social Work preferred, but not mandatory
- A minimum of one year of experience in a healthcare or hospice setting
- Valid driver's license and proof of insurance are required
