Role Summary
We are seeking a full-time Case Manager / Support Coordinator to join our team. In this role, you will manage a caseload of individuals with developmental disabilities, helping them access and maintain services in their communities. You'll serve as a vital advocate and liaison between individuals, families, providers, and state systems.
Key Responsibilities
- Coordinate and monitor services for individuals with developmental disabilities
- Conduct intake meetings and develop person-centered service plans
- Facilitate the initiation of services with approved state vendors
- Maintain monthly contact and ensure services align with individual needs
- Provide emergency on-call support as needed
- Accurately document and update information in the Consumer Information System (CIS)
- Bachelor's degree in Social Work, Psychology, Sociology, Counseling, Gerontology, Special Education, or Rehabilitation
- Minimum 1 year of experience working with individuals with intellectual/developmental disabilities
- Proficient in Microsoft Word and general computer applications
- Valid driver's license and proof of auto insurance
- BOGGS Support Coordination Orientation Certificate
