- Schedule and Shift Details: Monday- Friday 8a-5p, 1 weekend every 4-6 weeks, 2 holidays per year
- Travel: Ability to spend ~20% of your day driving to/from patient locations

Key Responsibilities
- Collaborate with team to provide management and delivery of patient care plans
- Meet critical deadlines for charting
- Support patients in Williamsport, PA and surrounding areas
- Bachelor's degree or higher
- Home Health (1+ years)
- 1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Practice Partner EMR
- EHR Implementation
- Valid Regular Driver License (DL) certification/license in the state of Pennsylvania (PA)
- Valid Physical Therapist certification/license in the state of Pennsylvania (PA)
