Your Role:

As a Nurse Practitioner, you will conduct thorough health assessments during in-home visits, lasting up to one hour. Your responsibilities will include:

  • Performing adult and geriatric assessments, including detailed medical history, diagnosis, and treatment plans.
  • Providing health education, referrals, and case management as per established protocols.
  • Collaborating with other healthcare professionals and family members to ensure comprehensive patient care.
  • Conducting point-of-care and lab testing as required.

Qualifications:

  • Master's Degree in Nursing or related field with NP licensure and prescription authority.
  • Board certification in a relevant specialty (AANP, AACN, or ANCC).
  • Current BLS, ACLS, or CPR certification.
  • Preferred experience as a Nurse Practitioner, but we welcome new graduates
  • Strong computer skills and excellent communication abilities.
