SALARY RANGE: 58.92 - 91.32HOURLY

General Summary:

Health Care Professional Responsible for safe, effective, timely medication distribution and administration.

Provides Specialty Pharmacy services, consistent with policies and accreditation requirements.

Responsibilities:

1. Medication/Solution Order Review - Reviews prescriber's orders for appropriateness relative to the patient's condition, height, weight, and age. Review prescriber's orders for appropriateness and compliance with legal requirements, institutional policies and procedures, and drug use restrictions in accordance with prescribing limits set by the Pharmacy and Therapeutics Committee and the Medical Executive Committee.
2. Medication/Solution Order Processing - Utilizes the full functionality, which includes allergy review, duplicate therapy, drug-drug interaction, dosage range, IV incompatibility and food-drug interaction screening of the automated order entry system to facilitate accurate and timely medication/solution distribution and administration. (Pharmacy Information System Competency)Guide - Utilization of the Pharmacy Order Entry System. Order entry processing transactions (includesnew, discontinued, update for IT, IV, and MED). Transactions based upon a monthly total reviewed fornumber of days worked in the calendar month. Quantifiable with HMM order entry statistics.
3. Intervention Documentation - Documents interventions using the approved documentation tools. (Includes Safety-First Reporting, Medication Errors/ Near-miss Reporting)Guide - # Interventions and Cost avoidanceGreater than 2500 interventions and cost avoidance $187,500 or greater per year = Far Exceeds2000 - 2499 interventions and cost avoidance $150,000 - $187,499 per year = Exceeds1500 - 1999 interventions and cost avoidance $112,500 - $149,.999 per year = Meets
