Job Summary
Position title: Care Coordinator
Job Summary: The Care Coordinator is responsible for building and maintaining strong relationships with patients, conducting in-person assessments, and developing individualized care plans in coordination with healthcare providers and caregivers. The ideal candidate will serve as a trusted advocate and central point of contact for patients, helping them navigate the healthcare system and access the resources they need to support their health and well-being.
Responsibilities
- Build and maintain trusted relationships with patients through ongoing communication and support
- Conduct face-to-face assessments to evaluate patients' medical, social, and emotional needs
- Develop personalized care plans in collaboration with providers, caregivers, and the care team
- Coordinate and connect patients with appropriate community resources to address social determinants of health
- Promote timely access to care, including preventive services and follow-up appointments
- Facilitate referrals to primary care and specialty providers as needed
- Assist with scheduling appointments and arranging transportation services
- Serve as the main point of contact and advocate for patients, caregivers, and the broader care team
- Educate patients on available health services and community support programs
- Maintain accurate and up-to-date documentation in accordance with organizational and regulatory standards
- Ensure high-quality, patient-centered care that improves outcomes and patient satisfaction
- Bachelor's degree in healthcare, social work, public health, or a related field preferred
- Associate's degree accepted with relevant experience in healthcare or social services
- Minimum 2 years of experience in a healthcare setting
- Prior experience in case management or care coordination is preferred
- Strong interpersonal and communication skills
- Ability to work independently and collaboratively within a care team
- Proficient in documentation and electronic health records (EHR) systems
- Knowledge of community health resources and patient advocacy
- Competitive hourly rate: $22-$25/hr
