As a Palliative Care Physician, you will manage high-risk patients with life-limiting illnesses, focusing on providing comfort, pain relief, and enhancing the quality of life. Your role is central in guiding high-risk patient care and facilitating smoother transitions between hospital and home care. While primarily delivering care in patients' homes, you may also conduct in-office visits based on individual patient needs. Successful palliative care requires teamwork and effective communication with various healthcare professionals, ensuring comprehensive support for our patients.
In this position, you will provide compassionate, effective care tailored to individual health challenges while being part of a dedicated team committed to delivering high quality, ethical, and culturally sensitive care.
Key Responsibilities:
- Direct and manage the transition of care for patients emerging from the hospital or skilled nursing facilities, collaborating with the interdisciplinary team to ensure a seamless continuity of quality care.
- Engage with patients to assess their health concerns, offering treatment that prioritizes comfort and alleviates symptoms related to chronic or serious illnesses.
- Coordinate with primary care physicians and specialists as necessary, ensuring a holistic approach to patient care.

Required Qualifications:
- M.D. or D.O. degree.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- New Mexico Pharmacy/DEA Registration or the ability to obtain it.
- Current medical license in New Mexico or the ability to acquire one.
Preferred Qualifications:
- Experience in Critical Care, Palliative Care, or Geriatrics.
- Background in a managed care environment, particularly with medically complex patients.
- Familiarity with Medicare Advantage Risk plans.
- Knowledge of HEDIS measures and experience in delivering value-based care.
The salary range for this position is competitive, with total cash compensation including base salary and bonuses influenced by local market conditions, experience, and performance.
