Job Requirements
  • Education Requirements: Valid physical therapy license in North Carolina
  • Certifications Requirements: BLS certification; ACLS certification preferred; Driver's License and proof of Insurance
  • Experience Requirements: Minimum of 1 year of home health physical therapy experience; Previous travel experience preferred; Experience with Homecare Homebase EMR; Geriatric and chronic disease management experience
  • Other Requirements: Ability to chart effectively using Homecare Homebase; Strong assessment and disease teaching skills; Flexibility and adaptability to home health settings; Valid driver's license; Proof of insurance; Ability to commute within a 50+ mile radius of the facility

Job Responsibilities
  • Assess patient conditions within home settings to develop personalized therapy plans aligned with physician orders.
  • Provide specialized physical therapy interventions such as wound care, IV therapy, and wound vac management to improve patient outcomes.
  • Document patient progress, treatments, and assessments accurately using Homecare Homebase EMR system.
  • Educate patients and caregivers on disease management, medication administration, and safety protocols.
  • Collaborate with interdisciplinary team members to coordinate care and optimize patient services in accordance with home health standards.
