The Director of Social Services utilizes core principles of care coordination to oversee the social work department. The role collaborates with an interdisciplinary team (including external providers) to coordinate care while advocating for the patient/family/resident and works to improve quality of care, resident/patient experience, and the health of individuals.  The primary focus is on social determinants of health that impact resident wellness.

What we are looking for:

We need a proven leader who will lead our social services team members and interact professionally with other Network leaders to support the daily living needs of our residents.  We are seeking someone who desires to develop trusting, caring relationships with both residents and co-workers. Our Social Services Directors are required to:

• Have a bachelor’s degree in social work from an accredited college or university and at least 2 years of post-acute or long-term care experience; A master's degree in social work and LSW/LCSW is preferred.
• Carry out supervisory responsibilities for the Social Services department in accordance with the policies of Presbyterian SeniorCare Network and all applicable laws.
• Oversee interviewing, hiring, and training of team members, including planning, assigning and directing daily work, appraising performance, rewarding and disciplining team members, and addressing concerns and solving problems.
• Possess strong interpersonal skills to support, advise, coach, and influence others.
• Have demonstrated knowledge of Point Click Care or other clinical software applications.
• Be proficient with PC and Microsoft Office applications, including Outlook, Excel, and Word. 

Social Work Licensure in PA is preferred.
