CPRC is a case management focused program that provides individuals with support to live an independent life in the community. Through a variety of multidisciplinary services that clients can choose from including medication management, therapy, group therapy, health and wellness support, employment services and transportation support, clients can develop skills, access resources and learn ways to manage their emotional and physical health.
What You'll Do
- Maintain and engage a caseload of clients seeking employment, building trusting relationships to help them achieve their vocational goals.
- Partner with clients to conduct individualized job searches and job development activities based on their strengths, needs, and interests.
- Teach and reinforce job-seeking and job-retention skills, providing coaching and ongoing follow-along support to help clients obtain and maintain employment.
- Connect with employers in the community to develop job opportunities and expand a network of employment partners.
- Gather and review client information to support individualized career planning and employment success.
- Assist clients in understanding how employment may affect benefits such as SSI or Medicaid.
- Transport clients as needed to support on-site or off-site job activities and employment-related appointments.
- Collaborate with a multidisciplinary team and community partners to coordinate care and share updates on client employment progress.
- 4-year degree in a social science field or 4 years of equivalent experience in lieu of degree.
- A valid automobile driver's license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work.
- At least two (2) years clinical experience in a direct service provision to children and adolescents in a mental health setting preferred.
- Working knowledge of mental illness and substance use disorders.
- Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.
- Excellent customer service skills with a recovery focus.
- Strong written, oral and interpersonal communication skills.
- Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as GSuite.
