As a Qualified Health Home Specialist, you will serve as a coach whose main focus is to support and assist clients in reaching their goals. They provide ongoing motivation, encouragement, and positive feedback when client makes constructive changes or progress. With the guidance of their supervisor and in collaboration with the BHH team, the Qualified Health Home Specialist is the primary entity responsible for the Health Promotion Services and assists the BHH Systems Navigator in coordinating care, serving as the secondary entity responsible for care coordination Services. Requirements: In order to be considered for this position, you must be qualified in at least one of the following ways: Must pass DHS background study Be in possession of a valid drivers license, auto insurance, and has reliable transportation Ability to drive a minimum of a 30-mile radius around assigned primary clinic location Must be proficient in the use of computers, specifically Microsoft Word, Excel, theuse of email.
