Responsibilities:
- Deliver home-based care to patients experiencing illnesses, injuries, and chronic diseases, often post-hospital discharge, under the supervision of a Registered Nurse (RN).
- Collaborate with an interdisciplinary team of physicians, RNs, Certified Nursing Assistants (CNAs), and aides to ensure quality patient care.
- Monitor patient vitals, perform assessments, document progress, and communicate changes to the RN supervisor.
- Assist RNs in implementing individualized plans of care and teaching patients self-care techniques.
- Educate family members about the patient's care plan, medication regimen, and home safety practices.
- Work closely with the interdisciplinary team to coordinate patient care and plan for discharge effectively.
Essential Qualifications:
- Graduate of a practical (vocational) nursing program for LPN/LVN.
- Current LPN/LVN license in South Carolina.
- A minimum of one year or 1200 hours of experience as an LPN/LVN.
