Job Details
Pt- home health needed asapPerforms a comprehensive Physical Therapy assessment of patients referred for service including but not limited to physiological, psychosocial, environmental factors and health history. Integrate pertinent data to develop a plan of care to promote function and safety. Participates with other staff in the development of agency policies and programs necessary to achieve agency objectives as they relate to patient care. Bachelor of Science Degree or Master's Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association.&bull
Current PT license in Mass. with one (1) year clinical experience and CPR certification required.&bull
Computer skills preferred.

Required Qualifications
Physical Therapist, Home Health Experience: 1 year Licenses: PT-MA Certifications: BLS-AHA(Copy needed) Holidays: occasional weekends & holidays Weekends: occasional weekends & holidays SSN Required DOB Required Work History: Should not be more than 30 days of gap in entire work history References: 1 Reference in entire work history Bachelor's degree or higher in Physical Therapy from an accredited Physical Therapy Program
