A Brief Overview
The Coordinator of the Orthopedic Joint Replacement Program is responsible for maintaining all the requirements for Certification by the Joint Commission. This position coordinates clinical program activities and maintains patient pathways and protocols. The coordinator leads interdisciplinary bi-monthly committee meetings and is responsible for patient education.
What you will do
- Leadership:
- Monitor and recommend improvements to the entire spectrum of care from the time patients enter the physician practice, through admission, SDS, surgery, 5 Marian, post-discharge
- Leads the development and expansion of Holy Name Medical Center's pursuit of Joint Commission certification in Advance Total Joint Replacement.
- Leads analyze and recommend changes to the methods and systems of the Total Joint program to improve the overall efficiency and effectiveness; identifying problem areas and recommending solutions.
- Provides professional leadership in working with the orthopedic staff, hospital personnel, referring physicians, and patients.
- Responsible for leading and organizing multi-disciplinary team meetings.
- Patient Care and Coordination:
- Acts as a patient advocate and educator, providing patient education, care coordination, and pre-operative optimization to prepare the patient for total joint replacement surgery.
- Facilitates and coordinates weekly education sessions for patients undergoing Total Joint Replacement.
- Round on all Total Joint patients, coordinating with Physicians, PA's and Nurse Practitioners to ensure comprehensive patient care.
- Introduce and promote the use of evidence-based clinical practice to establish processes, improve outcomes, and continually improve the orthopedic program to meet the demands of the changing healthcare continuum.
- Responsible for coordinating, running, and developing all forms of patient education strategies, in-person, video, online interactive, etc.
- Collaboration (Physicians/Interdisciplinary Team):
- Visit Physician practices working with office managers and physicians to develop, refine, and implement standardized protocols consistent with Advanced Total Joint Certification.
- Coordinate with physicians and their office managers on individual patient discharge dispositions to optimize patient care and outcomes.
- Works closely with the Quality department to decrease the length of stay and payor denials.
- Participates in various performance improvement committees including but not limited to Utilization Review Oversight Committee (UROC) and Clinical Documentation Steering Committee (CDSC) in order to utilize data to enhance patient care.
- Quality & Data:
- In coordination with Data Science, manage the collection, trending, tracking, and analysis of data to improve outcomes, based on evidence-based clinical practice to achieve, maintain and improve outcomes in the Total Joint Program to attain Advance Total Joint Certification and continue to move the program forward. Reports include but are not limited to:
- Surgical site infection
- Complications
- Re-admissions
- Joint Commission Certification
- CJR Bundles
- Provide expertise as a professional Registered Nurse with orthopedic clinical knowledge.
- Perform all other duties as assigned.
- Bachelor's Degree BSN preferred
- 1-3 years Surgical floor nursing experience required
- Excellent Communication Skills: Ability to effectively communicate with patients, families, and healthcare professionals.
- Critical Thinking Skills: Ability to assess patients, identify problems, and develop appropriate interventions.
- Must be able to coach, demonstrate and train
- Must maintain confidentiality of private health information
- RN - Registered Nurse - State Licensure and/or Compact State Licensure New Jersey RN License upon hire required
- BCLS - Basic Life Support through the American Heart Association (AHA) upon hire required
- CN-ORTHO - Certified Orthopedic RN upon hire preferred
- Disease-specific hip and knee certification upon hire preferred
