Licensed Practical Nurse

Purpose
To develop, coordinate, and implement programs and services for residents with cognitive impairments while creating a therapeutic environment that fosters the overall well-being of each resident's mind, body, and spirit.
Essential Functions
- Act as the charge nurse during shifts, managing care in the absence of the Director of Resident Care and Administrator.
- Administer medications according to physicians' orders with precision and care.
- Maintain clear communication with physicians regarding orders, services, and resident care needs.
- Implement and follow residents' individualized care plans as prescribed by physicians.
- Document resident services, changes in condition, incidents, and all follow-up actions accurately.
- Contribute to the development of personalized care plans and interventions for residents.
- Engage in collaboration with the interdisciplinary team and families to ensure quality care and services.
- Supervise certified nursing assistants and direct care aides to ensure optimal care delivery.
- Adhere to state regulations and uphold community standards in all aspects of care.
- Work alongside external professional health service providers for comprehensive resident care.
- Assist in coordinating engaging resident care activities and programming.
- Perform additional duties as assigned by the Administrator and/or Director of Resident Care.
Qualifications
- Current unencumbered LPN license is required.
- Applicants must be 21 years of age or older.
- Familiarity with applicable state regulations is essential.
- Ability to read, write, speak, and understand English is necessary.
- Experience working with older adults is highly valued.
- Strong communication skills with residents and their families are crucial.
- Maintain professionalism in conduct with colleagues; supervisory experience is desirable.
- Must possess the physical strength to lift and assist with moving items weighing more than twenty-five pounds.
- Ability to squat, reach, stretch, and tolerate extended periods of walking and standing is required.
