Reporting Relationship:
- Reports directly to the Administrator.
- Serves as Social Service designee, maintains social service MDS as required by the state, provides social service information as needed for IDPH survey.
- Open and prepare new charts for Skilled Nursing, Assisted Living and Memory Enhancement.
- Collect New Admission Paperwork for business file.
- Conduct Assisted Living Orientation for new members/families moving in.
- Completes Skilled Nursing contracts as necessary.
- Attends staff meetings and other meetings as requested, including safety squad, QA, Medicare and care plans.
- Participates in the required in-service training.
- Pre-authorize insurance as needed.
- Participates in On-Call rotation for scheduling/call in phone; evenings and weekends.
- May assist in taking pictures of all new admissions in Prairieview and input into PCC.
- Support Community Outreach Coordinator for tours as needed with prospective members and families throughout Prairieview.
- Maintain various electronic and paper PV admissions documents.
- Update and/or create PV admissions documents, including PDF versions that may need to be emailed.
- Attach electronic documents to Sherpa, per Community Outreach Coordinator, as needed.
- Update Assisted Living Admission Checklist and Orientation Packets, including printing and replenishing paper copies.
- Update PV New Member(s) paperwork distribution check list, including printing and replenishing paper copies.
- Create AL/ME contracts for new PV move-ins.
- Update Schedule of Fees section of PV Contracts annually for current years fees.
- Update, print and bind PV Member Handbooks and Skilled Nursing Welcome books.
- Make copies of appropriate PV admission documents for new move-ins and distribute to appropriate team members prior to new admissions or move-ins.
- Update Member Access Database including PV form, financial and billing information with pertinent information from the various PV Admission documents.
- Follows established standards of excellence for The Garlands and implements facility policies and procedures.
- Identifies safety hazards and initiates corrective action.
- Reports any possible cases of Abuse immediately to the Administrator/Director of Nursing.
- Follows Abuse allegation protocol as directed by Administrator.
- Provides respect, privacy and dignity to all Prairieview members.
- Investigates all door alarms immediately.
- Performs all duties as assigned in an effective, cooperative, professional and timely manner.
- Comes to work as scheduled and consistently demonstrates dependability and punctuality.
- Dresses in professional attire that represents The Garlands standard of excellence.
- Demonstrates excellent listening and communicates effectively with patience and understanding.
- Writes clearly and concisely.
- Maintains confidentiality
- Demonstrates knowledge of The Garlands entire facility and directs potential members appropriately according to their needs.
- Applies computer and organization skills in the development of spreadsheets, memos, forms, and mailings that are accurate, timely, and of a quality level that is reflective of The Garlands image.
- Completes Incident Reports accurately and timely.
- Completes Concern Reports accurately and timely.
- Prepare and process invoices for Administrator.
- May assist in maintaining Prairieview at The Garlands social media pages.
- May provide administrative support for Administrator and Director of Nursing.
- Must be a flexible, highly customer service and people oriented individual who demonstrates compassion, patience and understanding and is willing to do whatever it takes to provide the highest level of service.
- Must be well organized with the ability to handle a variety of responsibilities and changing priorities, while maintaining an upbeat, customer focus.
- Must be detail-oriented and proactive, with the ability to take initiative, follow through and complete responsibilities with minimal supervision.
- Must be able to work a flexible schedule, including holidays.
- Must possess highly developed verbal and written communication skills including excellent telephone etiquette.
- Must be able to demonstrate resourcefulness and the ability to research appropriate options to meet client needs.
- Experience having worked in a hotel and/or senior housing is a plus.
- Knowledge of word processing, spreadsheets, and e-mail software applications is required.
- Must be able to tolerate long periods of walking, standing, bending and/or sitting.
- Must be able to lift up to 50 lbs
