In this role as a Physical Therapist in Home Health, you will conduct assessments and evaluations of patient care needs, focusing on mobility, endurance, pain, and various medical conditions. Your expertise will help shape effective treatment plans aimed at enhancing and improving our patients' overall well-being.

Key Responsibilities:

  • Evaluate patients' functional status and identify their physical therapy needs in collaboration with physicians to create effective therapy plans.
  • Deliver services within your professional scope as determined by state laws, ensuring adherence to care plans and collaboration with the healthcare team.
  • Monitor and communicate patients' responses to treatments, as well as any changes in their condition, to supervising nurses and physicians.
  • Provide treatments aimed at alleviating pain and maximizing functional capacity, utilizing established best practices.
  • Educate patients and their families and caregivers about the care plan, functional limitations, and progress towards therapeutic goals.
  • Visit patients as outlined in the treatment plan, document progress during each visit, and submit notes in accordance with LHC policies.
  • Complete all patient evaluations and develop the Physical Therapy plan of care following state-specific guidelines.
  • Report on evaluation outcomes, set goals, and project anticipated frequency of care.

Qualifications Required:

  • Current Physical Therapy licensure in your state of practice.
  • Valid CPR certification.
  • Current driver's license and vehicle insurance; access to a reliable vehicle or public transportation.
  • Ability to work effectively in diverse home situations, regardless of clients’ backgrounds or conditions.

Compensation will vary based on local labor markets, education, experience, and certifications. We offer a comprehensive benefits package including recognition programs, stock purchase options, and 401k contributions, all subject to eligibility requirements. The hourly pay for this role ranges from $34.23 to $61.15 for full-time positions. We comply with all minimum wage laws as applicable.
