Job Type: Contractor | Hourly
Hours: 40 hrs/week | Day Shift (8:00 am - 4:30 pm)
Shift Rotation: 1 Saturday every 5-6 weeks

Job Summary

The PT Case Manager is responsible for coordinating and managing therapy services for home-based patients requiring physical therapy as their primary clinical need. This role involves comprehensive patient evaluation, development and implementation of individualized care plans, and ongoing assessment of outcomes. The PT Case Manager serves as both a therapist and a care coordinator to ensure optimal patient progress, safety, and quality outcomes across the patient caseload.

Key Responsibilities

Clinical Assessment & Care Planning
  • Conduct thorough patient evaluations to identify functional status, home safety needs, and therapy goals.
  • Develop, implement, and adjust individualized patient care plans in collaboration with the patient, family, and physician.
  • Establish measurable, patient-centered goals and ensure progress toward outcomes.
  • Reassess patients periodically and update documentation as required.
Care Coordination & Communication
  • Coordinate therapy services, referrals, and interdisciplinary care with physicians, social workers, nursing staff, and other providers.
  • Monitor and document patient response to the care plan, adapting interventions to changing clinical needs.
  • Communicate effectively with patients and caregivers about treatment plans, expectations, and therapy education.
Scheduling & Caseload Management
  • Manage a caseload of home care patients, prioritize visits, and adjust schedules to support efficient care delivery.
  • Optimize daily scheduling to support productivity and best-practice utilization.
Documentation & Compliance
  • Maintain complete, accurate clinical records consistent with organizational and regulatory standards.
  • Ensure adherence to clinical protocols, safety standards, and professional practice guidelines.
Quality Improvement
  • Identify patient care trends, performance improvement opportunities, and best-practice initiatives to enhance outcomes and operational efficiency.
Qualifications

Required:
  • Bachelor's or Master's degree in Physical Therapy.
  • Licensed Physical Therapist (PT) in the State of Wisconsin.
  • Valid driver's license and reliable transportation for home visits.
  • Strong clinical assessment, communication, and organizational skills.
Preferred:
  • Doctorate in Physical Therapy (DPT).
  • Experience with case management, home health operations, or hospice populations.
  • Certification in case management or home health therapy specialization.
Work Environment & Expectations
  • Hybrid model combining office, remote documentation, and patient home visits.
  • Travel throughout the North Wisconsin service area.
  • Collaboration with interdisciplinary care teams to improve patient outcomes.
  • Participation in quality assurance and professional development initiatives.
