Licensed Practical Nurse (LPN) - Full Time

Assignment Duration

This is a 13-week contract position.

Schedule

Shift: Full Time Days and Evenings, 5x8 schedule per week. Every other weekend and holiday required.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care, ensuring the needs of adult and geriatric patients are met. The LPN must demonstrate knowledge and skills appropriate to the age-specific needs of patients.

Responsibilities

  • Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
  • Contribute to the development of individualized care plans reflective of nursing diagnoses and interventions.
  • Evaluate resident outcomes and collaborate with patients and families to ensure comprehensive care.
  • Provide mentorship and orientation to new staff members.
  • Maintain current knowledge and competency in nursing practices.
Required Experience / Certifications / Licensure
  • Current New York LPN Licensure.
  • Basic Life Support (BLS) certification (AHA or RQI only).
  • Minimum of 1 year experience as an LPN, preferably in a long-term care facility.
