Certified Social Worker- Full Time | Willow Creek Behavioral Health | Green Bay, Wisconsin
About the Job:
The Social Worker I plans, directs, defines and coordinates individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance abuse and physical and emotional abuse. Conducts individual and group therapy, crisis intervention, case management, and education.

Roles and Responsibilities:

• Conducts social history assessment of the patient through observation and patient/family interview, obtaining information on the patient's needs, skills, interests and limitations for the development and implementation of the treatment plan.

• Coordinates and plans programs and activities to meet patient's social and emotional needs.

• Promotes recovery, symptom reduction and the achievement of the highest level of social functioning.

• Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle.

• Participates in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge plan.

• Communicates or aids family members in understanding and supporting the patient.

• Manages a caseload and maintain accurate and timely documentation to assure patient records comply with facility and accreditation standards.

• Facilitates education, process groups, support groups and referrals as requested.

• Assists in the development of effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient.

• Bachelor's degree in Social Work from an accredited college or university required.

• Actively pursuing a master's degree in social work.

• One or more years' experience working with patient population preferred.
