Your Role as a Primary Care Physician
The Primary Care Physician (PCP) at ChenMed plays a vital role in delivering value-based, outcomes-focused care to a panel of up to 450 patients. Responsibilities include aiding patients in achieving their health goals, fostering trust to improve their health outcomes, and leading care teams to enhance performance goals.
Key Responsibilities:
- Provide independent care for patients with both acute and chronic illnesses.
- Take full accountability for patient care and outcomes, engaging with specialists as needed.
- Actively participate in patient hospital visits and coordinate care with hospitalists.
- Lead a dedicated care team that includes medical assistants and care coordinators.
- Support care coordination for patients who are homebound or in facilities.
- Contribute to center management and assist in patient recruitment.
- Oversee the actions of Advanced Practice Practitioners (APPs) in collaboration.
Qualifications:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Current State Medical License or eligibility to obtain one.
- Board certification in relevant specialties preferred, Board Eligibility required.
- Current DEA number and Basic Life Support (BLS) certification.
