Job Description

We are seeking a qualified Social Worker II to provide comprehensive social services to patients and their families within an acute care setting. The ideal candidate will support discharge planning, psychosocial assessments, crisis intervention, and coordination of post-acute services while collaborating closely with interdisciplinary healthcare teams.

Required Qualifications

Master’s Degree in Social Work from an accredited institution

Active LMSW or LCSW license (state of practice)

Hospital-based social work experience preferred

Strong assessment, counseling, and care coordination skills

Ability to work effectively with diverse patient populations

Preferred Qualifications

Previous experience in acute care or inpatient hospital settings

Experience with high-risk psychosocial cases and crisis intervention

Knowledge of community resources and post-acute care placement

Experience working with pediatric and adult patient populations

Key Responsibilities

Provide social services to patients and families for assigned medical services

Conduct psychosocial assessments and discharge planning evaluations

Counsel patients and families regarding illness, treatment, emotional stress, and social challenges

Perform crisis intervention for high-risk cases, including abuse or neglect situations

Act as a liaison between patients, families, hospital staff, and external agencies

Coordinate referrals to community resources and post-acute care services

Assist with patient placement or transfer to post-acute facilities

Advocate for patients related to informed decision-making, end-of-life care, and ethical concerns

Utilize specialized interviewing and communication techniques with children when applicable

Maintain accurate documentation and participate in care planning discussions

Support supervision or mentoring of students or newly hired staff as needed

Licensure & Certifications

Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
