PT- home health needed asap 2+ acute nursing care (hospital base) and Home Care experience BLS (AHA) Valid Driver License required for this need Must be able to work independently with minimal supervision/assistance Acute care experience preferred Certified home health experience including proficiency with OASIS required and NON-NEGOTIABLE AT LEAST TWO SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW MUST BE ADDED TO TRIO/ PROFILE - PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION.
