Responsibilities
As a Nurse Practitioner, you will conduct annual health risk assessments in patients’ homes or via telehealth, within your scope of practice as designated by a Collaborative Physician.
Key Duties Include:
- Conduct in-depth in-home health assessments to identify active and chronic illnesses, as well as physical, mental, and social needs at the time of evaluation
- Gather patient history, conduct examinations, and establish diagnoses
- Document patient encounters in compliance with NCQA standards
- Measure vital signs as necessary
- Coordinate case management referrals and communicate effectively with the primary care physician as needed
- Engage with patients, caregivers, agency nurses, other healthcare providers, and vendors to ensure thorough diagnosis and care
- Adhere to OSHA Universal Precautions while performing clinical duties
- Maintain patient confidentiality at all times
- Participate in mandatory meetings, in-services, and requested committees
- Engage in ongoing professional development and maintain necessary licenses and affiliations

Qualifications
Required Knowledge, Skills, and Experience
- Active, unrestricted nurse practitioner license
- Board certification from ANCC, AANP, or NCCPA
- Current BLS certification
- Active enrollment in Medicare/Medicaid
- Valid driver’s license with a good driving record
- Proficient EHR skills
Preferred Qualifications
- Experience or training in geriatrics
- Strong teamwork skills and ability to build effective relationships with patients and healthcare professionals
Conditions of the Role
- Ability to adapt to various weather conditions and heterogeneous residential environments
- Full physical capability to handle and lift patients, including manual dexterity, coordination, and normal sensory functions
- Regularly lifting/carrying items weighing up to 50 pounds
- Comfortable traveling in a vehicle for up to 150 miles daily in both urban and rural settings
