Join Our Passionate Team as a Hospice Social Worker

Are you ready to make a real difference in the lives of patients and their families? We are seeking a compassionate and skilled Social Worker (MSW) to join our dedicated hospice team. This is a full-time position based in Renton, WA, serving the King County area.

Salary: $40 - $47 per hour plus mileage reimbursement

Schedule: PRN as needed with on-call support

Important: Must possess active LICSW licensure in the state of WA.

What You Will Do:

  • Help the hospice team recognize and address the social, spiritual, and emotional issues impacting the patients’ health, fostering their dignity and well-being.
  • Collaborate as part of an interdisciplinary team to develop and review comprehensive care plans for patients.
  • Assess the social and emotional needs of patients and families and prepare them for coping with terminal illness.
  • Educate families about advance directives and planning to ensure informed decision-making.
  • Leverage community resources to support patients and their families through their journey.
  • Contribute to our bereavement program to help families after a loss.
  • Assist in discharge planning and transitions in care as necessary.

Qualifications:

  • Master's Degree in Social Work from an accredited institution.
  • Current state licensure as a Clinical Social Worker (LICSW) is required.
  • At least one year of post-graduate experience in a healthcare setting; two years preferred.
  • Experience with Medicare/Medicaid/Insurance regulations is highly desirable.
  • Background in grief counseling and discharge planning is a plus.
