Psychiatric-Mental Health Primary Care Provider

As a Primary Care Provider in Psychiatric-Mental Health, you will work under the supervision of the Medical Director, partnering with PACE Physicians and the IDT to deliver high-quality assessments, treatment plans, and ongoing medical education for our participants.

Key Responsibilities:

• Provide psychiatric evaluations, consultations, and treatment plans for participants, collaborating closely with PACE Physicians and Community-Based PACE Physicians.

• Conduct comprehensive psychiatric assessments, either in-person or via telehealth, alongside a physician and work with the IDT to develop tailored care plans for each participant.

• Assist in discharge planning for participants transitioning from acute or long-term care environments in collaboration with physicians and the IDT.

• Refer participants to psychiatrists as necessary while maintaining clear communication with the PACE Physician and Community-Based PACE Physician.

• Oversee participant care within nursing homes, ensuring regular visits that meet both WelbeHealth standards and individual participant needs.

• Act as a consultant for the IDT, communicating participant status changes, collaborating on care plans, and coordinating around-the-clock care delivery.

Qualifications:

• A Master’s Degree in Nursing or a Physician Assistant program with a focus on Psychiatric/Mental Health from an accredited institution.

• Certification as a Psychiatric Mental Health Nurse Practitioner or Physician Assistant.

• A valid, unrestricted State License as a Psychiatric Mental Health Nurse Practitioner or Physician Assistant.

• State CDS Certification and DEA License.

• NPI License.

• Access to reliable transportation.

• A minimum of two (2) years of experience working with elderly or frail populations.

Compensation ranges between $145,399 and $174,478 annually, along with potential bonuses. Actual pay will depend on experience and qualifications.
