Psychiatric Mental Health Nurse Practitioner

At Central City Concern’s Old Town Clinic, you’ll join a tight, multidisciplinary team delivering integrated primary care, mental health support, and substance use treatment for people navigating some of the toughest barriers in Portland. This role lets you practice meaningful psychiatry in a setting where collaboration, compassion, and equity guide the work every day.
What You’ll Do
- Provide psychiatric evaluation, consultation, and ongoing medication management in a primary care environment.
- Deliver psychopharmacological and brief psychotherapeutic interventions… including motivational interviewing.
- Support recovery by prescribing and managing medications such as buprenorphine, naltrexone, and acamprosate.
- Respond to crises, conduct suicide assessments, and coordinate care during emergencies.
- Consult with behavioral health and primary care colleagues to support integrated treatment planning.
- Engage with clients from diverse backgrounds and help connect them to culturally responsive services.
- PMHNP licensure in Oregon, national certification, and full prescriptive authority with a DEA number.
- 3–4 years of PMHNP experience is highly preferred.
- Strong clinical judgment in psychiatric and SUD treatment, plus comfort working with complex, vulnerable populations.
- Ability to thrive in a fast-paced, integrated care setting while supporting colleagues and clients with professionalism and empathy.

Schedule: Monday-Friday 8:00am - 5:00pm
Location: Old Town Clinic
Salary Range: $164,000-$213,000 *This range is influenced by professional experience, licensure/certifications, and internal equity.
Minimum Qualifications:
- Bachelor’s degree in nursing and Master’s degree in Psychiatric/Mental Health Nursing from State Board of Nursing-approved schools of nursing.
- Current license to practice nursing in Oregon and certified as a PMHNP by the Oregon State Board of Nursing and the National Certification.
- 3-4 years of experience practicing as a PMHNP HIGHLY PREFERRED
- Must meet CCC privileging requirements as required by FTCA.
- Current BLS certification required prior to start.
- Bilingual in Spanish preferred.
- Must have full prescriptive powers in Oregon with a DEA number.
- Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
- Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
- The OAR prohibits SUDS program staff from having, within the previous 3 years, a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
- Must be able to work within an integrated, multidisciplinary setting.

- Work as a member of the Old Town Clinic multi-disciplinary team to deliver comprehensive, client-centered and trauma-informed integrated services.
- Provide direct patient care including but not limited to: psychiatric evaluation, consultation, psychopharmacological and psychotherapeutic interventions, and monitoring and addressing adverse effects.
- Document plan for psychiatric treatment and on-going care in the primary care outpatient setting.
- Provide suicide assessment and crisis intervention.
- Conduct care of psychiatric/mental health emergencies and initiate arrangements for continuing care or referral to appropriate facility.
- Provide brief psychotherapy interventions including motivational interviewing.
- Provide patient and family education.
- Provide clinical consultation to staff in behavioral and primary care disciplines, including consultation with other CCC practitioners.
- Prescribe and manage medications to support substance use recovery including buprenorphine, naltrexone and acamprosate
- Actively participate in the integration of services at OTC and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
- Proactively identify opportunities for improvement within Old Town Clinic workflows and logistics. Participate in review of Old Town Clinic Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
- Build understanding of and appreciation for diversity among OTC clients and support their connection to culturally responsive and culturally specific services as requested.
- Participate in clinical staff meetings and case conferences in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients.
- Refer patients, as necessary, to other mental health care providers and community resources/services.
- Participate in records review to ensure that services are clinically appropriate and effective.
- Maintain client records as required by program standards and funding requirements.
- Participate in compliance and quality improvement activities as directed.
- Participate in training opportunities to advance skills and knowledge in behavioral and primary care.
- Work within scope of training and experience and seek consultation when appropriate.
- Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
- Attend all mandatory CCC trainings in a timely manner.
- Perform other duties as assigned.
- Ability to consider the impacts and outcomes for underserved communities during decision-making process.
- Ability to consider impacts of oppression, structural racism, and individual bias on client outcomes.
- Knowledge of evaluation plans and treatment of clients with a variety of psychiatric disorders, with psychopharmacological and psychotherapeutic interventions.
- Knowledge, evaluation and treatment planning for clients with substance use disorders, including both pharmacotherapies and psychotherapies.
- Skills in providing patient care, including rapidly building rapport.
- Ability to make treatment decisions based on assessment of patients' concerns within PMHNP scope of practice.
- Ability to provide nonjudgmental and compassionate care to vulnerable, complex populations and to seek supervision as necessary
- Ability to practice independently and seek consultation when appropriate.
- Ability to delegate patient care, as needed, and if appropriate.
- Ability to work in a busy clinic setting and responsively attend to unpredictable requests for assistance from colleagues.
- Ability to successfully interact with diverse populations, including clients and colleagues.
- Sufficient manual dexterity and physical ability to perform assigned tasks.
- Ability to maintain accurate records and necessary paperwork
- Ability to remain calm and composed when dealing with clients who are in crisis.
- Ability to maintain strict confidentiality of patient information.
- Excellent communication skills including diplomacy, tact, effective “bedside manner”
