The County of Riverside is seeking a Bilingual Community Health Worker/Promotore to join our team. We are currently recruiting for a Community Health Worker/Promotore I/II to support our Family resources locations in Desert Hot Springs and Mecca.

The incumbent will be responsible for driving throughout Riverside County to attend various event fairs, representing and offering programs across the region. They will play a key role in connecting with community members to help them access essential health services and resources, including assistance with health program applications. Responsibilities will include, but not limited to, conducting outreach and interviews to assess community needs while providing tailored support to individuals and families, particularly in underserved populations, and provide resources between the public and health/social services to promote health and well-being programs offered with the County.

Competitive candidates will possess a Community Health Worker certification, strong communication skills, and a solid understanding of programs and community outreach within healthcare and human services programs, and high volume-case management.

The level at which the position is filled is based on applicants' qualifications and at the department's discretion.

EXAMPLES OF ESSENTIAL DUTIES
• Build rapport with clients and gather, share, and assess accurate information to identify efficient and effective opportunities for intervention utilizing strategies that reflect the client's values, attitudes, and beliefs.
• Provide health education, instruction, and information in a manner that clients can easily understand and address barriers to physical and mental health services.
• Assist clients in navigating the healthcare system and public health programs by phone and in person to help clients successfully participate in their medical and/or behavioral care.
• Engage with community members to raise awareness about violence prevention strategies.
• Build trusting relationships with individuals and families affected by asthma, offering guidance, emotional support, and practical assistance.
• Connect program participants to community resources necessary to achieve healthy outcomes; address healthcare barriers and identify and address health-related social needs; make closed-loop referrals to local human service systems to support clients address their needs including but not limited to housing, food, personal safety, transportation, childcare, energy assistance, education, income assistance, and education.
• Provide care management services for clients at highest risk of poor health outcomes by helping them access clinical and community services necessary to implement the client's care plan.
• Provide transitional care services (TCS) for clients transferring from one setting or level of care to another and maintain close contact with clients and caretakers both in-person and on the phone.
• Support clients' engagement with their primary care physician (PCP); identify and connect clients to services that address Social Determinates of Health (SDOH) needs; promote wellness and prevention, help members manage their chronic diseases, and support efforts to improve maternal and child health.
• Participate in all team collaborations and continuous quality improvement initiatives and projects.
• Ensure documentation in electronic health records (and other tracking and monitoring systems) are accurate, useful and in compliance with regulatory requirements and accreditation standards.
MINIMUM QUALIFICATIONS
Community Health Worker/Promotore I
($40,830.97 - $54,407.81 Annually)

Education: Graduation from high school, possession of a Certificate of Proficiency issued by the California State Board of Education or attainment of a satisfactory score on a G.E.D. examination.

Experience: Any combination of lived experience working with diverse populations in the community (e.g., women with infants and young children, homelessness, substance abuse, persons involved in the justice system, AIDS/HIV patients, older adults, and underserved populations) which would provide an opportunity to acquire the knowledge and abilities listed herein. Normally, this would include, one year of experience which has provided personal first-hand knowledge of the problems, needs, attitudes, and behavior patterns of the community as they relate to public health.

Other Requirements
License: Possession of a valid California Driver's License may be required.

Completion of a Department of Health Care Access and Information approved CHW/Promotore (Community Health Worker) training program and possession of a CHW/P/R certificate is required within 12 months of hire.

Community Health Worker/Promotore II
($43,076.80 - $57,399.88 Annually)

Education: Graduation from high school, possession of a Certificate of Proficiency issued by the California State Board of Education or attainment of a satisfactory score on a G.E.D. examination.

Experience: One year of experience as a Community Health Worker/Promotore I or equivalent non-County classification.

OR

Experience: Two years of lived experience, that is equivalent to that of a Community Health Worker/Promotore I working with diverse populations in the community (e.g., women with infants and young children, homelessness, substance abuse, persons involved in the justice system, AIDS/HIV patients, older adults, and underserved populations) which would provide an opportunity to acquire the knowledge and abilities herein.

Knowledge of: Client populations, community resources, techniques for group and individual interviewing and counseling; basic recordkeeping principles and practices; and computer equipment and applications related to the work.

Ability to: Address the client population's unique needs in a culturally sensitive manner; relate and communicate effectively with the community served; lean and understand the organization and operation of the assigned department and of outside departments and agencies; maintain strong working relationships with a wide range of community agencies and organizations; obtain and record accurate information for case documentation and other reports.
Other Requirements

Completion of a Department of Health Care Access and Information approved CHW/Promotore (Community Health Worker) training program is required.
Must possess and maintain a CHW/P/R (Community Health Worker) certificate. Some positions may require additional specialized certification.

MISCELLANEOUS RETIREMENT: County of Riverside has three retirement Tiers through the California Public Employees' Retirement System (CalPERS).
  • Tier I (Classic Member - Formula 3% @ 60): Applicable to current and former County of Riverside local miscellaneous employees hired prior to 08/24/2012 and did not withdraw CalPERS contributions. The employee contribution is eight (8%) percent.
  • Tier II (Classic Member - Formula 2% @ 60): Applicable to local miscellaneous employees 1) hired after 08/23/2012 through 12/31/2012; 2) Previously employed with another CalPERS contracting public agency or a reciprocal retirement system, with a break in service of less than six months between the separation date with the previous employer and the appointment date with the County of Riverside. The employee contribution is seven (7%) percent.
  • Tier III (PEPRA New Member - Formula 2% @ 62): Applicable to CalPERS local miscellaneous new members hired on or after the implementation of the Public Employees' Pension Reform Act of 2013 (PEPRA) which took effect January 1, 2013. Effective July 1, 2023 employee contributions are 7.25% and will increase to 7.75% effective July 1, 2024.

A new member is defined as any of the following:
  1. A new hire who enters CalPERS membership for the first time on or after January 1, 2013, and who has no prior membership in any California Public Retirement System.

10 or more years = 160 Hours (20 Days)

Maximum Vacation leave accumulation is 480 hours.

SICK LEAVE: Four (4) hours Sick Leave accrual per pay period with unlimited accrual.

HOLIDAYS: Normally 12 paid holidays per year

BEREAVEMENT LEAVE: Allowed 5 days (3 days are County paid; 2 additional days can be taken from accrued Sick Leave balance)

BASIC LIFE INSURANCE: Equal to one times annual base salary not to exceed $50,000 of term life coverage. Premiums are paid by the County. Additional Supplemental Life plan is available for employee purchase.

DISABILITY:
Employees contribute to State Disability Insurance (SDI), which provides wage replacement benefits for those unable to work due to non-work-related illness, injury, or pregnancy. SDI also includes Paid Family Leave benefits. The wage replacement rate, as defined by current law, is between 60-70% of wages earned during the SDI eligibility period. Benefits can be coordinated with other available leave balances to provide up to 100% of regular pay.

POST RETIREMENT MEDICAL CONTRIBUTION: A monthly contribution is made by the County towards retiree health insurance offered through the County as governed by the applicable Memorandum of Understanding.
