Practice Details: Successful Primary Care group hiring Location: LaPorte, Texas Outpatient Quality Care / Managed Care In Office -M-F:8a-5p No weekends Low patient panel Great Work/Life balance Comfortable with use of Electronic Medical Records (EcW) Detect and manage chronic diseases Develop patient strategy to reduce hospitalization and ER visits Collaborate with Case Managers and Specialty Providers Educate patients on the value of compliance Compensation & Benefits: Competitive base salary of $230,000"$290,000 (depending on experience) Robust bonus structure, plus generous sign-on and relocation incentives Comprehensive benefits package Community: La Porte offers easy access to scenic waterfronts, parks, and recreational amenities, including Sylvan Beach Park and other natural attractions along Galveston Bay.
