  • Coordinate and manage individualized care plans across various disciplines, ensuring continuity and quality of care for all assigned patients.
  • Serve as the primary point of contact for patients, families, and care teams regarding treatment plans, progress, and follow-up needs.
  • Monitor and evaluate patients' progress and make recommendations for changes in care plans as needed.
  • Facilitate communication between physicians, nurses, case managers, and other healthcare professionals to optimize outcomes.
  • Oversee discharge planning to ensure smooth transitions from acute care to home or other healthcare facilities.
Candidate requirements A Bachelor of Science in Nursing (BSN) is required; a Master's degree in Nursing or Healthcare Administration is preferred. Minimum of 3 years of clinical nursing experience, with at least 1 year in care coordination, case management, or a related field. Strong knowledge of care transition processes, discharge planning, and healthcare resources. Basic Life Support (BLS) certification is required. Case Management Certification (CCM) or Care Coordination and Transition Management (CCTM) certification preferred.
