Join our team as a Part-Time MDS Coordinator, where your expertise will make a significant impact in the lives of our residents. In this role, you will:

  • Plan, schedule, and ensure completion of all components of the RAI process for each resident according to regulations.
  • Conduct comprehensive assessments using medical records, patient interviews, staff interactions, and electronic records.
  • Utilize specialized software for completing and transmitting RAI components.
  • Ensure assessments are completed accurately and in a timely manner.
  • Collaborate with the interdisciplinary team (IDT) to plan and review care plans.
  • Complete CAAs that assist in the development of care plans synchronized with MDS assessments.
  • Participate in meetings to assess changes in resident status, facilitating necessary assessments, including Significant Change assessments.
  • Maintain a schedule for OBRA and PPS assessments, communicating regularly with the IDT team.
  • Work jointly with the IDT team on resident discharges from admission through to discharge.
  • Assess residents to determine appropriate diagnosis coding and enter codes into the software.
  • Complete and modify diagnosis listings and codes based on patient changes.

Requirements:

  • Licensed Nurse in good standing; RN preferred.
  • Acceptable background check.
  • Must be over 21 years of age.
  • MDS experience preferred.
  • Ability to read, write, comprehend, and communicate effectively in English.
  • Strong organizational skills and the ability to work independently with integrity.
  • Excellent interpersonal skills and ability to tactfully interact with people in various settings.
  • Proficient with office machines and computer applications.
  • Ability to make independent decisions and possess problem-solving skills.
  • Sound judgment and discernment are essential.
  • Familiarity with medical symbols, abbreviations, terminology, and long-term care regulations.
