Our Primary and Palliative Care Nurse Practitioner Will:
  • Participate in Advanced Care Planning collaboration with residential community, family members, and Care Navigators.
  • Practice independently within Nebraska's full practice authority scope, conducting comprehensive patient assessments, developing individualized care plans, and providing expert symptom management and pain control to improve quality of life for both patients and their families.
  • Collaborate with interdisciplinary team members including nurses, social workers, and chaplains, as part of a dedicated care team.
  • Maintain their own patient panel and manage care decisions autonomously.
  • Thoroughly document patient care activities in electronic health records.
  • Provide education and support to patients, families, and healthcare team members.
  • Participate in an on-call rotation as required.
Requirements

Our Primary Care Nurse Practitioner Has:
  • A master's degree in nursing.
  • A current Advanced Practice Registered Nurse (APRN) license in the state of Nebraska.
  • The ability to obtain Iowa licensure.
  • A current DEA certification.
  • 1-2 years of clinical experience as a Nurse Practitioner.
  • Experience caring for the geriatric population in a primary care setting (preferred).
  • Experience with Assisted Living requirements, rules, and regulations (preferred).
  • A Hospice and Palliative Care certification (preferred).
  • A current BLS/ACLS certification.
  • An active driver's license, acceptable driving record, and access to reliable transportation.
