What Makes This Role Stand Out:
- Consistent Weekend Hours: Fit work around your life with weekend-only scheduling
- Excellent Pay: $200 per Start of Care visit + mileage
- Focused Role: Specialize in new patient admissions and care plan initiation
- Community-Based: Get out of the office and into the homes of clients across the metro
- Professional Independence: Use your clinical judgment to help patients begin their care journey
- Perform weekend Start of Care (SOC) admissions with complete OASIS documentation
- Develop and initiate care plans in collaboration with the interdisciplinary team
- Evaluate patient needs and coordinate necessary services
- Educate patients and caregivers on disease management and home care best practices
- Valid Minnesota RN license and CPR certification
- Proven experience in home health and OASIS documentation
- Reliable transportation, driver's license, and active insurance
- Strong communication and clinical assessment skills
